Entre Hermanos is currently looking for an enthusiastic bilingual (English/Spanish,) professional and experienced individual to assist people living with HIV maintain program eligibility and access social and medical resources. The Non-Medical Case Manager will work as part of an interdisciplinary team at Entre Hermanos, assisting people living with HIV to improve their lives by providing assistance in four areas:
(1) Provide HIV/STI education and prevention,
(2) to ensure patient access to an HIV specialist,
(3) to ensure patient access to HIV treatment and
(4) to ensure patient access to stable food, shelter and transportation.

SUMMARY
Under general supervision, as part of a multi-disciplinary team, the non-medical case manager will listen to their needs and create an action plan to fulfil them. The non-medical case manager will provide services relating to improving retention in care for this specialized subset of patients. Provide education and referrals for people who required it. Ensures eligibility for a variety of medical and social services and assists patients in completing appropriate paperwork. They may provide assistance in accessing social, community, legal, financial and other needed services. They may perform a range of administrative functions required to ensure efficient program operation.
DUTIES AND RESPONSIBILITIES
Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services for Case management programs.
Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services;
Works closely with medical case managers and peer navigator to reduce patient barriers to obtaining needed medical care and /or social services;
Assist with the identification of community resources that build support for medically and socially complex patients;
Maintain current information on all regularly accessed community resources for Case Management
Perform a range of administrative functions required to ensure efficient program operation.
Maintain professional growth and development through completion of required departmental and Washington State Department of Health competencies.
Perform miscellaneous job-related duties as assigned.
MINIMUM JOB REQUIREMENTS
High School diploma, GED or 3 years of directly related experience… bilingual (English/Spanish).
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Strong interpersonal skills and the ability and desire to work effectively with a wide range of constituencies in a diverse community.
Ability to effectively communicate information in a manner easily understood by the client.
Ability to gather data, compile information, and prepare reports.
Excellent organizational skills with the ability to prioritize duties.
Knowledge of community health care and vocational services and resources.
Fluent in Spanish and English writing and speaking.
CONDITIONS OF EMPLOYMENT
Washington driver’s license and ability to drive using own vehicle for community visits with mileage reimbursed at standard rates.
Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).
Required N-95 mask fitting, testing, vaccinations to include annual TST, Tdap, and Hepatitis B will be determined based on location and nature of position.
FULL TIME ( 40HRS/WEEK)
WORKING CONDITIONS AND PHYSICAL EFFORT
Work is normally performed in a typical interior/office work environment.
No or very limited exposure to physical risk.
No or very limited physical effort required.
Home, hospital & nursing home visits may be required.
