Entre Hermanos is looking for an enthusiastic and dedicated people who are passionate about linking those living with chronic conditions to long-term, stable housing while improving housing retention skills and building positive landlord relationships.

Housing program assistant Duties/Responsibilities:
§ Carry out case management services: develop supportive relationships, assess for housing needs, create service plans, and monitor participant progress.
§ Assist with housing searches to obtain stable, safe housing for individuals with housing barriers.
§ Coach participants on housing retention skills and coordinate services for improved housing and financial stability.
§ Complete housing program eligibility renewals.
§ Network to create a working knowledge of local housing resources, understand the impact of the current housing crisis in the Region, and the complexities of housing barriers.
§ Provide support and education on the Landlord/Tenant Laws and advocate for participant’s rights as tenants, respond to complaints and develop positive liaison relationships with landlords and housing authorities.
§ Collaborate with medical case managers to facilitate client care and resolve short-term needs.
§ Maintain timely, accurate and confidential client files in accordance with program requirements.
§ Participate in appropriate Entre Hermanos’s committees or activities to promote quality improvements.
§ Perform other duties as assigned.
Education and Experience: Must be Bilingual ( SPANISH/ENGLISH )
§ Experience providing case management (or similar) services to people with high barriers to care.
§ Preferred Bachelor’s degree in a social services or related field; equivalent experience is acceptable.
§ Lived experience: individuals from non-dominant cultures encouraged to apply.
